What is the comman thought on when to replace these items? Is it a scheduled replacement or replace if defective? My manual says inspect and replace every other year. Does that sound right? Engine is a 5.0 Gi Volvo Penta.

There are two bellows. One for the exhaust which is optional. I have a friend with a 190 Regal that doesn't have one installed. It came that way from Regal.

As to the other bellows, the one that protects the drive shaft, that one needs to be water tight and replaced ASAP if compromised. If water gets in there, you will quickly damage your gimbal bearing and start to damage the universals. As to replacement, the universals only need replacment if they are bad, ditto for the gimbal bearing and the bellows itself.

My 288 Vista has the original bellow, universals and gimbal bearings on both drives. 300+ hrs at 2.8 yrs of ownership.
