I Hate that sound. 85 hours on my starboard engine. #1 hole exhaust gasket? This is what we found. Had the manifold decked 9 thousands to get a flat surface.

Strange; the gasket was clearly misaligned, but there seems to have only one hole for the bolt/ stud, but there's an indentation corresponding to a hole in the manifold or head for the other one. Were there not 6 bolts holding on the exhaust manifolds like there are on my 2004 GM derived 5.7 ? Should there have been 6 and they left one out because there was no hole in the gasket and they couldn't screw it in ?

Having built and raced small block Chevs for more than 20 years - looks to me like the manifold gasket was simply misaligned last time the manifold was put on and blowout occurred where the gasket was folded. A 9 thou warp should not be a problem - these composite gaskets will easy take that up. Root cause - poor installation of the manifold.

A lot to be said for using studs (especially at the ends) for installing the manifolds rather than just bolts as the gasket can sit on the studs, then the manifold placed on - no trying to juggle things with 3 hands! With bolts you are trying to line up the gasket and the manifold and the bolt all at the same time - this is how this would have happened.

yup ....

Shoddy assembly at the manufacturer ... the second bolt would not even go in, so it was left out! (it would seem).

Just a OOPS at the factory. There is only 4 bolts one bolt on each end and two in the middle. See Link

Wow, I'm surprised they designed the manifold mountings like that with only 4 bolts, when there are 6 tapped holes in the head.

Ahhh ...... I have a bit more leeway for that then. Still not good, from the assembly line standpoint. The assembler would have known that that one bolt went in different from normal (forcing it thriough the gasket).

Coincidentally, I can certainly see it happening ...... I worked on the engine assembly line at Ford, in the '70s ......... installing left-side exhaust manifolds on 390s, of all things.
