1. I can't hear it at all when the boat is running out of the water on the hose.
2. Can hear it easy in the water and makes a kinda low rotational knocking kinda sound.
3. Forward, reverse or neutral....it doesn't matter.
4. It is not coming from the motor.

I thought maybe it was the exhaust bellows but I looked and it seems ok. Could it be the gimball?

It might be the u-joints, as LouC said. Or it might be gear backlash in the top end of the outdrive from wear. Usually these causes are more pronounced when the boat is up to operating temperatures, and when the boat is in neutral.

Does it make a different noise when you make turn either to Port of Starboard??

In my experience that would indicate a U-Joint that has been previously mentioned.

Could be the gimbal bearing. How do the bellows look? If water got into the bellows the bearing isn't going to last very long.

You'll have to pull the drive to check. At the same time, you'll be able to look over the u-joints as well.

Please make a video, upload it to YouTube and share it with us. Might be easier for us to tell and at the very least, it could help someone in the future.
